About the Direct Support Professional (DSP) Questionnaire

The Direct Support Professional (DSP) Questionnaire is intended to complement the organization’s Organizational Self-Assessment (OSA) results by providing the workforce perspective. Direct Support Professionals (DSPs) are directly impacted by their organization’s recruitment and retention efforts. Completion of the DSP Questionnaire will provide an additional layer of understanding where an organization’s strengths lie and determine where they should concentrate their efforts related to workforce recruitment and retention.

The assessment was made available on April 5th, 2023

The contact person who completed your organization’s OSA will receive an email with a link and QR Code to the DSP Questionnaire. The link and QR Code should then be passed along to the DSPs within your organization. The Questionnaire needs to be completed by May 2, 2023.

Why should your organization participate in the Organizational Self-Assessment?

  • It’s important to hear from the workforce that is directly impacted by the organization’s current recruitment and retention efforts.
  • Each provider organization will have their own data and comments, no one else. And yet you can compare yourself to the region or state data that will be provided in an aggregated report.
  • Hearing and comparing perspectives will help your organization learn what’s working and what’s not working in your efforts.
  • Understanding where your organization is will help guide your organization to the next steps.
  • Beyond this grant, recommendations will likely be made around positive systems change that you will want to be part of.

Instructions for Completing the DSP Questionnaire:

  1. We strongly recommend that you view the webinar, Introduction to the Workforce Recruitment and Retention Technical Assistance Project: Information for DSPs, prior to starting the DSP
  2. Enter your responses into Qualtrics by clicking here or by scanning the QR code below.
  3. You have an option to enter a raffle upon completion of the Questionnaire. We are giving away several $150 gift cards to show our appreciation for your participation in this project. (Your contact information will not be linked to your responses)
